What We Treat

Shoulder pain treatment in Richardson.

The reach for the seatbelt you now do with the other hand. The side you stopped sleeping on. Most shoulders get better without surgery, and it starts with a cause-first evaluation.

Rotator cuff Impingement Frozen shoulder Talk to a Specialist

Sound Familiar?

Does this sound like you?

  • Reaching across your body for the seatbelt because that arm won’t go back there anymore
  • Waking at 2am because you rolled onto that side in your sleep
  • A catch about halfway up when you reach for the top shelf, right in the same spot every time
  • Getting a jacket on one sleeve at a time, in a specific order you worked out months ago
  • Asking someone else to lift the bag into the overhead bin
  • Dropping overhead pressing out of your workout and telling yourself you never liked it anyway

Two or more? That’s a pattern, not bad luck.

Why It Happens

Your shoulder trades stability for range. Then something gives.

The shoulder moves more freely than any joint in your body, and it pays for that with a small margin for error. It stays put because the rotator cuff and the muscles around your shoulder blade keep the ball centered while your arm does the work. When one of those falls behind, the ball drifts, and the tendons and bursa underneath get pinched every time you go overhead. Do that a few thousand times and you get impingement, bursitis, or a cuff tendon that frays. Sometimes it is a tear or a labral injury. Sometimes the capsule itself thickens and locks down, which is what frozen shoulder is. Often there was never one dramatic moment at all, just posture, repetition, and an imbalance nobody caught.

That is why a cause-first evaluation matters here. We test range of motion, strength, joint mobility, nerve involvement, and posture, and watch how the shoulder blade moves when you lift. Two shoulders with identical pain can need opposite plans. Guess wrong and you spend months strengthening the thing that was never the problem.

Person holding a painful shoulder

The Honest Truth

A shoulder you stop using is a shoulder that stops working.

If you keep waiting

  • You avoid the painful range, the capsule tightens around what’s left, and the stiffness becomes its own problem
  • The cuff keeps getting weaker, so every overhead reach pinches a little harder than the last one
  • Your neck and upper back take over the work, and now two things hurt instead of one

If you start this week

  • You find out whether this is a cuff, a capsule, or a shoulder blade problem, because the plans are not the same
  • Hands-on treatment starts at that first visit, not after a stack of paperwork
  • You give conservative care a real shot before anyone starts talking about injections or surgery

You built the workarounds so well that nobody noticed, including you. That is not toughness failing. That is a shoulder quietly getting smaller, and it’s worth taking back.

How We Fix It

Four phases. Built around your shoulder.

You work one-on-one with a licensed physical therapist, every visit. Here’s what the four phases look like for a shoulder that has been guarding itself for months.

Starts visit one

Pain

Get the night pain out of the way.

Joint mobilization, soft tissue release, and trigger point work settle the irritated tissue under the pinch, plus the neck and trap tension that came along with it.

Once the pain settles

Prime

Open the range back up.

Hands-on mobility work and graded movement give you back the arc you have been steering around, especially reaching behind you and overhead.

When motion returns

Perform

Rebuild the cuff that let go.

Rotator cuff strengthening and scapular stability training, loaded progressively so the joint gets stronger instead of just getting used again.

Before you’re cleared

Prevent

Fix the mechanics that pinched it.

Overhead mechanics get retrained and tested under load, with the ergonomic and home program work that keeps it from creeping back. That’s the endpoint, not another appointment.

Larry and Erica worked on strengthening my shoulder. I did not even realize how bad it had gotten until I started building it up with their focused approach. Within weeks, I had more mobility and am able to use my shoulder fully without pain! Highly suggest!

Where it starts: getting your arm through the range without the pinch

Where it ends: overhead, loaded, and sleeping on that side again

Questions, Answered

What people ask before booking for shoulder pain.

It is a fair question, and usually it is not the first step. Rotator cuff wear shows up on scans of shoulders that feel perfectly fine, so an image alone does not tell you whether the tear is the reason you cannot sleep. The evaluation does more of that work: range of motion, strength testing, joint mobility, and how your shoulder blade moves when you lift. Most shoulder problems respond to conservative care, and starting it does not close any doors. If your findings point to imaging or a surgical opinion, we will tell you straight and help you get there.

Loading a shoulder correctly is how it gets stronger, not how it gets damaged. Resting it completely is what lets the capsule tighten and the cuff fade, which is the road to a stiff shoulder. The difference is in the dose and the direction: which range, how much weight, how often, and what to back off when it flares. That is decided at your evaluation and adjusted as you improve, one-on-one with a licensed physical therapist, every visit. You will never be handed a generic sheet of exercises and sent off to guess.

An irritated cuff and a shoulder that has already stiffened up are different jobs, so we will not quote a number before we have seen yours. What you get is a plan with an endpoint. Your shoulder moves through four phases, Pain, Prime, Perform, and Prevent, with a clear target for each one, so you always know where you are and what is left. In Texas you can be seen by a licensed physical therapist for up to 30 days without a physician referral, so you can get the answer this week.
