Scope of this policy

This policy explains how On Point Movement & Performance handles information collected through this website. It covers the pages you browse and the forms you choose to submit here.

It does not describe how patient health records are handled once you begin care with us. If you have questions about your records, contact the clinic directly and ask to speak with our office staff.
